Hi!

Thanks for sending us your picture/video. Before we will consider using it in the movie, we need you to fill out a Release Form (linked below) and return it to us. Here's what you need to do:

1. Print, fill-out and sign a Release Form.


a. If you are under 18, you MUST use this "MINOR" Release Form and have your parent or legal guardian fill it out and sign it: Click Here to download.

b. If you are 18 or older, use this "ADULT" Release Form: Click Here to download.

2. Return the signed Release Form to us by email/fax AND mail:

b. Mail your signed form to:
